(Hereinafter - "Program" or "Program "Support ") - a program is designed to help patients with acute and chronic diseases in areas such as cardiology, pulmonology.

The Patient Support Program is organized by the international pharmaceutical company AstraZeneca (AstraZeneca), which is represented in Georgia by Astrazeneca UK Limited Representation in Georgia (hereinafter - "Program Organizer").

The program aims to: increase patient awareness in relevant therapeutic areas, reduce the cost of therapy with medications included in the program.

The program is valid only on the territory of Georgia.

Participants of the program can be any individual who has reached the age of 18 and is fully capable, who has been prescribed by a doctor with a medicine which international non-patented name is included in the program. For individuals who have not reached the age of 18 or do not have full legal capacity, participation in the program is possible only through their legal representatives.

The program does not apply to drugs other than AstraZeneca. To find out if the medication is included in the program, enter the prescribed drug name in the search.

Registration or participation in the program does not involve the payment of any fees for the registration or participation in the program.

Delivery of the Beginner Package and Program Participant Card to an individual who has been prescribed a treatment by a doctor which international non-patented name is included in the program is free.

The program owner reserves the right to unilaterally change the terms of the program and/or the rules of participation in the program, change the program of action and/or medication included in the program and suspend any prior notice to program participants. Terminate the program, at your sole discretion and at any time.

1. Terms of use of the program participant card

Individuals who have been prescribed the drug by the doctor - the card cannot be used again or for other commercial purposes.

Registration is required to participate in the program and to use the program participant card. Unregistered cards of the program participant do not allow the use of the program. You can learn about the registration procedure on a separate registration page.

Upon registration in the program, the program participant receives a personal account. You can see the functions of the personal cabinet on a separate page. When registering for the program, as well as for the program, the data provided by the person must be reliable and authentic.

Inaccessibility of data or inaccurate / false data will not allow you to participate in the program. The decision on data inaccuracy / invalidity is made by the program administrator. In the event that the Program Administrator finds data that is incorrect / false, in the opinion of the Program Administrator, or is suspicious, the Program Participant Card may be blocked from use and canceled by the Program Administrator without notice to the Program Participant.

The program participant card is personal. It is not possible to transfer the program participant card to third parties. The program administrator must be notified of someone else's card.

The program administrator should be notified as soon as possible of the loss or damage of the program participant's card.

The medicines included in the program can be purchased at a reduced cost in accordance with the terms of the program at any pharmacy participating in the program. In order to purchase a medicinal product included in the program at a discount included in the program, the program participant must submit the relevant prescription (according to the current law) and the program participant card. The program participant pharmacy employee must scan the program participant card barcode.

To reduce the risk of misuse of the program participants' cards, the maximum number of medicines included in the program, which can be purchased by one program participant using the monthly participant card, is limited to: no more than 3 (three) packages. One medicinal product per month and at the same time not more than 9 (nine) packets of all medicines included in the program, during the month.

The Program Participant Card is the property of the Program Organizer.

1. Termination of participation in the program

A registered program participant may terminate his / her participation in the program at his / her own request upon request by the program administrator. Upon receipt of such a request, the program participant card will be canceled and the use of the card will be unavailable.

Individual participation in the program may be terminated by the program administrator, with the corresponding cancellation of the program participant card, in the absence of transactions (purchases) using the program participant card, for a period of more than 24 (twenty four) months. Termination occurs without prior notice to the Program Participant

The Program Administrator may unilaterally terminate the participation of any Program Participant by appropriate cancellation of the Program Participant Card without any notice if, in its sole discretion, the Program Administrator determines that a Program Participant has violated any provision of the Terms and Conditions. Or that the Participant Card is used in violation of any provision of the Terms of the Program, or the Participant Card is used for misuse or fraud or for misleading or other suspicious or illegal purposes.

1. Limitation of Liability

The Program Administrator is not responsible for any losses, losses, lost opportunities or benefits, any other negative consequences, directly or indirectly:

Suspension, temporary suspension, termination of the program, as well as any changes to the program, its terms and conditions;

Acceptance of program access or failures/interruptions (including when using the Program Participant Card, accessing any of the Program services or resources) for any or all Program Participants;

Software or hardware malfunction, defect, error that affects the usability of the program for any or all program participants;

Recovering information that is lost or potentially distorted or damaged as a result of a technical malfunction, circumstances beyond its reasonable control, or any unlawful action by any third party;

Illegal actions of any third party, in particular, but not limited to fraud, falsification, etc.

Circumstances beyond reasonable control, including but not limited to: natural disasters, riots, hostilities, etc.

1. Personal Data and Privacy

The processing of personal data provided by the individual program is carried out in accordance with the privacy policy.

The program is valid by the program organizer until its completion. The program organizer reserves the right unilaterally, without any prior notice to the program participants, to change the terms and conditions of the program and / or the rules of participation in the program, to change the program of action and / or the list of drugs. In the program and suspend / terminate the program in whole or in part, at your own discretion and at any time by registering with the program, program participants agree to receive communications about the program based on the contact information of this participant. Each participant of the program has the right to refuse to receive such communications at any time by notifying the program administrator.
